Required Documents for Member:

  • Patient's (above 21 years old) Medicaid copy
  • Patient's address, phone number, and his/her emergency contact (name + phone number)
  • Patient's family doctor’s contact (name + phone number)

Required Documents for Caregiver:

  • Social Security Card
  • Employment Authorization Documents
  • Physical Report (within 6 month): Drug Screen, MMR, PPD, COVID Vaccines Record
  • 2 Reference Letters
  • Proof of Address (e.g.: Utility bill)
